Fox Subacute recognizes the great need for alternative programs for today's medically compromised patients. Fox has developed Models of Care and offers subacute programs for the management of ventilator-dependent patients. We recognize that the best road to recovery for these patients is an environment with special care in an alternative setting. We believe that setting should be outside the hospital, in facilities where the focus is on the management of individual patients and their pulmonary care of vents and other healthcare needs. We also recognize that Fox Subacute Centers serve as one of the few alternatives for such patients in the Tri-State region.

Since 1985, Fox Subacute Centers and Fox Nursing Homes have focused on the total person, utilizing a trained medical/rehabilitation team of professionals working together to provide the highest level of quality care. Each patient and family is part of a skilled interdisciplinary team that includes: nurses, physicians, dieticians, respiratory, physical, occupational, speech and recreational therapists, as well as social services and case managers. Medical management of the patient is addressed aggressively, while the quality of life and the spiritual-psycho-social component of the individual are considered priorities. Program design is two-fold: a quality healthcare delivery model offering aggressive medical management and a financially responsible approach.

At Fox, we believe good communication is essential in order for the patient to be an active participant in the recovery process. Our admissions coordinators/case managers assure that all patient and family questions and concerns are answered completely and promptly.

To alleviate any added stress, the Fox coordinators/case managers work closely with the patient's insurer by managing benefits while the patient is being cared for at our centers. The internal admissions and case management process inter-faces with the managed care referrer, assuring early intervention for transfer and coordination of benefits with a continued treatment plan at Fox.
