About Fox Subacute Centers
Fox Subacute, the first skilled nursing facility in the country to convert traditional nursing homes to sophisticated, subacute care facilities that exclusively service the post acute (outside the hospital) ventilator dependent patient. We specialize in providing progressive ventilator weaning, rehabilitation services and other subacute care such as dialysis, TPN and wound management. We have gained a stellar reputation as the industry leader in caring for the respiratory patient since 1985.
Our facilities are conveniently located in Plymouth Meeting, Warrington and Mechanicsburg, Pennsylvania. Our facilities are licensed by the PA Department of Health and Fox Subacute at Clara Burke, the Plymouth Meeting facility and Fox Subacute at Warrington have both been accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).
Our highly trained team will ensure that our patients receive proper respiratory care and total systems care based on their own specific needs. We pride ourselves in upholding the highest standards of caring for the vent patient while meeting state and federal regulations.
Due to our experience in caring for the ventilator dependent, medically complex patient outside of the hospital Fox Subacute has developed a model of care that has proven to be the best road to recovery with quality of life.
We focus on the whole patient, utilizing a skilled medical/rehabilitation team of professionals working together with the patient and family to provide the highest level of quality, individualized care. Medical management of the patient, including weaning is addressed progressively while the quality of life and the spiritual-psycho-social component of the individual are also considered priorities.

At Fox, we believe good communication is essential for a good outcome; therefore we utilize the Interdisciplinary Patient Care Conference along with our admissions coordinators/case managers to assure that all patient and family questions and concerns are answered completely and promptly. To alleviate any added stress, The Fox admissions coordinators/case managers work closely with the patient’s insurer by managing benefits while the patient is being cared for at our centers.
