Admissions
For questions about our services provided or to make a referral to our facility, please contact Amy Swartley, RN, Director of Admissions at 610-862-0643 or by email at aswartley@foxsubacute.com, or you can fax your referral to 888-224-8104. Admissions for all three of our buildings are centralized through Amy. This allows for more bed availability, so please specify which facility you are interested in. If there is no bed available at the requested facility we are able to accept the patient at another location and then transfer him/her to the requested facility when a bed becomes available.
At Fox Subacute, we have two clinical liaisons in the Philadelphia region, who are available to evaluate ventilator patients in an acute care setting. In the Harrisburg region, we have an admissions coordinator, who is also available to evaluate ventilator patients in an acute care setting.

We are willing to negotiate with insurance companies that we are not contracted with.
